The ElderCompass Navigation Plan is a customized and prioritized senior care plan. We serve families, caregivers and professionals advocating for Elders as they seek to understand and initiate health care considerations.

To help you determine whether an assessment can be helpful for you or your loved one, here are some questions to answer:

  1. Is there concern about memory loss or confusion?
  2. Have there been frequent falls?
  3. Has there been a diagnosis for which you want a second opinion?
  4. Are medications ineffective or difficult to manage? Are you concerned about the dosages?
  5. Has there been a decline in the ability to do something that previously was done well?
  6. Has there been an unexplained weight loss or gain?
  7. Has there been a sudden change in you or your loved one, such as a withdrawal from usual activities?
  8. Has there been an unexplained personality change?
  9. Have there been problems with urinary control?
  10. Is your Elder unsafe when alone?
  11. Do you feel that you or your loved one can no longer manage the household but want to continue living at home?
